How Difficult is It to Disappear, Move to Another Location, and Start Life Over?

I am not planning to do it (!), but I was wondering, with all of the modern tracking and investigative methods available today, whether it is really possible anymore, and what are things a person should or should not do in order to accomplish his objective of disappearing, moving to another location, and starting life over.
